If you have severe asthma, prednisone has probably helped you at some point. It works, it works quickly, and during a bad flare it can keep you out of hospital. Nobody is suggesting otherwise.
But there is a cost that builds quietly, and it starts earlier than most people expect.
The effects add up over a lifetime, not just during a course. Research following more than 30,000 people taking oral steroids found increased risk across every complication studied — beginning at a total lifetime dose of around 500 mg. That is not decades of daily tablets. It is a handful of short courses.

What builds up
Adrenal glands. Your body's own steroid production can slow down when it is supplied from outside. This is the single largest risk in the research — and it is why you must never stop prednisone suddenly.
Bones. Thinning and fracture risk rise even at relatively low total doses.
Blood sugar. Risk of type 2 diabetes rises with several courses a year.
Infection. Higher risk of serious infection and shingles while taking it.
Mood and sleep. Depression and anxiety are more common than most people realize.
Eyes, stomach, skin, heart. Cataracts, ulcers, easy bruising and raised blood pressure all feature.
This one matters: Should you ever stop prednisone suddenly on your own?

Answer: No, never. Your body may have reduced its own steroid production. Stopping abruptly can be dangerous. Any change must be planned with your care team.
This is not a reason to fear prednisone
During a flare, it is the right tool. It is a reason to ask a different question at your next appointment: is there a way to need it less often?
For many people, the answer is yes. Newer treatments have allowed large reductions in steroid use — in some studies, cutting the dose by 70 to 75%. Some people come off maintenance steroids altogether.
